How do I forward consent forms?
Go to Consent Forms, click on Forward Consent Form, enter the details needed- make sure the email address is correct, and click Send.
How do I check consent forms submitted by my clients?
Go to Consent Forms, click on your client’s name, then click Consent Form from the popup selection.
How can I delete a consent form?
Go to Consent Forms, click on your client’s name, then click Consent Forms from the popup selection. Look for the form you want to delete, click the three dots beside it and click Delete.
How can I print a filled-out consent form?
Login to your account in facesconsent.com. Go to Consent Forms, click on Client Consent Forms then select the PDF version button. A new page will show where you can save, and print out the consent form.
Can I edit a completed consent form?
Yes, just go to Consent Forms, click on your client’s name, then click Consent Forms from the popup selection. Look for the form you want to edit and click it.
Can a client complete a consent form during a face-to-face consultation?
Yes, you can have your client complete a consent form on the app during a consultation or before treatment. Just click Consent Forms > Create Consent Forms and fill out the consent form with your client.
