Prescribing Clinic at your location in Coventry and surrounding areas. Minimum of 2 clients. 1. Select a clinic date and time 2. Book your clients in/send out forms as usual 3. Select Shelley Evatt as your prescriber to order your POMs 4. I will sign off the scripts when I see your client for consult 4. Pay your prescriber/pay for the products as usual 5. Prescriptions must be used within 6 months of issue, once issued/after this time- further treatments require a new assessment/script
